1.  Item information that can be found on the product detail page such as item code, item title, qty, price, size and color (if applicable).

2.  Credit card information (Visa, Master, or AE).  Your name as appeared on the credit card, full credit card number, expiration date, security code, and billing address.

3.  Contact information that includes your email and phone number.

4.  Select the shipping and insurance option you want.
